If the issue persists, others have found that if Lara is allowed to become infected on Day 9, then healed during the morning of Day 10, her reflections will appear as intended.

It must be a mistake
ACHIEVEMENT_SOMETHING

Stop pointing fingers
ACHIEVEMENT_THEY
On day 7 as the Bachelor, complete "The Mask and the Raiment" sidequest, and wear both the Talon Mask and Executor's Cloak at once. This quest and the main quest on day 7 have an odd interaction - to be able to begin this quest by speaking to Mark Immortell at the theatre, you must have given the inquisitor the papers from the executors, but not have completed the main quest yet in its entirety.
Now you know where to look
ACHIEVEMENT_LOOK
As the Haruspex or the Bachelor, see every play. It's possible to unlock this achievement by going back with a save file if you miss a play, and plays from both playthroughs count (i.e. if you see all plays except the Day 5 play as the Bachelor, watching the Day 5 play as the Haruspex will earn you the achievement).
This, on the other hand, is your fault
ACHIEVEMENT_MIGHT
On day 12 as the Bachelor or Haruspex, simply die.
Just a little bit more
ACHIEVEMENT_HELLO_2
On day 12, heal another character's bound and then pick their ending over yours.
Notes[]
The words in the descriptions of the secret achievements spell a sentence, writing "Hello, hello, don't look up, they might notice something", giving a nod to both the developers and The Powers That Be both in and out of game.
