“ | Modernity is a very active and aggressive element that’s always seeking to grow its influence. That’s why we have to level it out with “fluid things” wherever possible. We can’t let through a roleplayer who’s wearing sneakers. If authentic footwear isn’t an option, you can settle for something nondescript made of leather. | „ |
— From the Pathologic 2 artbook |
Clothes are equippable items in Pathologic 2. They can be equipped to boost resistances against physical damage and infection, and cloaks can also be used to upgrade the Haruspex's inventory. All clothing items can be purchased from Tailors or looted from houses (houses in burned districts apppear to spawn clothes more often), however certain clothing sets will not be available in stores until a certain point in the game. Very rarely, items of clothing can be looted from houses prior to their formal introduction to stores.
Each clothing category excels in defending against one of the four major sources of plague, which are as follows:
- Masks protect from plague clouds.
- Cloaks protect from contact with diseased people.
- Gloves protect from infected surfaces.
- Footwear provide improved protection from infected air in infected districts.
- Cloaks, masks and gloves also protect from passive infection, but much less than footwear.
Articles of clothing will degrade faster when subjected to their respective variant of infection damage. As an example, because footwear has a heightened resistance against infected air, it will degrade faster than other articles of clothing when moving through infected districts.
Articles of clothing from the same quality tier have identical physical resistances. Clothing items lose durability whenever taking physical- or plague-based damage. Both physical and disease resistances stack by sum.

Cothurni |
Protective footwear. Helps one rise above their troubles—and the poisoned earth. Cothurni—unusual footwear that elevates the wearer high above the ground—are popular with the local theatre. It's likely the orderlies owe their awe-inspiring height to them. |
Equipped in the Feet slot. Very high protection from infection, medium damage resistance. |
Executor Cloak |
Protective garment. The pendants serve no obvious purpose, yet it loses its protective qualities if stripped of them. Orderlies—the volunteer medical assistants—use the costumes borrowed from the local theatre as protective cloaks. This is the traditional costume of the Reaper, an allegory of Death. |
Equipped in the Torso slot. Very high protection from infection, medium damage resistance. |
Executor Mask |
Protective headwear. Replaces all smells with that of the healing herbs placed in the beak. The mask of Muu Shubuun, "the wicked bird"; part of the Reaper costume from the local theatre. Impossible to see through, it's actually mounted above an actor's head, making them peek through a hole in the cloak. |
Equipped in the Head slot. Very high protection from infection, medium damage resistance. |
Gauze Mask |
A basic protective item that covers the face. Common among medical personnel, this protective mask accumulates infection fast, quickly becoming a hazard in and of itself, and muffles speech. Still, better than nothing. |
Equipped in the Face slot. Moderate durability and resistance. |
Shoes |
Worn and unimpressive. I've worn down three pairs of shoes getting here… and nearly as many running trivial errands for the local rulers. Ugh. |
Equipped in the Feet slot. Low durability and resistance. |

"Clothes" is the second of five tabs in the player's inventory. This tab holds items that can be worn on the player's head, hands, torso, and feet. Clothes may provide blow softening, protection from bullets, protection from fire, and/or protection from infection. Clothes can be mended by women.
All resistances of clothes stack by sum. Note: physical resistance protects from fire, fire resistance is additional.

Army Boots |
Protect the feet and lower legs from both infection and wounds, including stab wounds. Make it safe to walk over burning coals and sharp objects such as nails. | Feet |
Army Gloves |
Strong army-issue gloves. The fingertips and striking surfaces are reinforced with leather inserts. The fabric is an excellent repellent. They protect the hands, wrists, and forearms from contact with infected surfaces, as well as from burns and wounds. | Hands |
Disposable Gloves |
Decrease the chance of accidental infection, especially when handling infected bodies. It is recommended to change gloves regularly. Cannot be re-sterilized. | Hands |
Drapery |
A piece of light fabric (a sheet, a tablecloth, or a cotton curtain). Used for moving around in infected areas in order to prevent accidental tactile contact with infected particles and surfaces. As a single-use item, it's usually thrown away before entering a building. | Torso |
Executor's Cloak |
Part of a theatre costume. Worn nowadays by "deathbringers"—corpse carriers, undertakers, watchers, and others who handle dirty work. The cloak draws attention but removes any desire to talk with its wearer. | Torso |
Overalls |
Army-issue sanitary overalls. An excellent repellent. Protects the torso, hands, legs, and head from accidental contact with infected surfaces. The durable material of the overalls protects from stab wounds and burns. Its camouflage colouring makes it difficult to target the wearer with a firearm. | Torso |
Protective Gloves |
Light gloves made of synthetic material that serves as a good repellent. Protects the hands from contact with infected surfaces, and moderately against burns and grazing stabs. | Hands |
Protective Mask |
An antibacterial mask. Reduces the chance of accidental infection, especially from conversation with an infected person. It is recommended to regularly change the mask. A used mask cannot be re-sterilized. | Head |
Repellent Ankle Boots |
Short boots. The material is a good repellent. Protects the feet and ankles from contact with infected surfaces. Provide no protection against blows and rat bites. | Feet |
Repellent Cape |
A light cape made of synthetic material that serves as a good repellent. Protects the torso from accidental contact with infected surfaces. Provides moderate protection from fire. It's believed that the muted coloring of the cape makes the wearer a difficult target for firearms. | Torso |
Repellent Cloak |
A long cloak made of synthetic material that serves as a good repellent. Protects the torso, wrists and neck from accidental contact with dangerous surfaces. The material is somewhat flame-proof, protecting the torso from burns. | Torso |
Repellent Jackboots |
Strong tall jackboots. The material is a good repellent. Protects the legs and ankles from contact with infected surfaces. Provide little protection against blows and dog bites for the lower legs and ankles. | Feet |
Talon Mask |
A theatrical mask shaped like a bird's head. Stands for "deception" and "traps" in mime performances. Such masks are worn by those who wish to remain incognito. Executors put aromatic antiseptics inside the beak. Provides no protection from infection. | Head |
