These minor characters may be encountered or interacted with at different points throughout the game. Some of these encounters may change or can even be missed altogether depending on the player's actions.

Aide (Оруженосец) is a child who belongs to the Soul-and-a-Halves and helps the Haruspex the first time he becomes infected with the Plague. He calls the Haruspex Cub and tells him their dogs found him on the ground.
Aide says the reason the air is all black is due to pollution from the factory, and that his Dad says it used to pollute only the water, but now the air is polluted too. When asked about his Mom, Aide says that she says nothing, just prays, and that his aunt Dora saw the same things five years ago. Aide also talks about his half, a chaffinch, and that it chose him when he was there. He mentions that he hasn't seen it in a year.
Aide then gives the Haruspex an Immunity Booster given to him by Notkin.
Aide is a variant of the Swift NPC.

Apostate (Отступник) is the name given to the Worms who lived in the Termitary and are members of the Kin that initially rejected the Haruspex.
Should the Haruspex choose to heal the schism within the Kin without violence (which requires surviving the Abattoir), he will find Apostate just outside the stairs that serve as a side access to the Termitary, on the same floor as Taya's chamber. Apostate will question the Haruspex about how he plans to lead the Kin. Satisfied with the Haruspex's answer, Apostate concedes and will go to Shekhen with the rest of the Kin.
Similarly to the other Worms at Shekhen after the Kin's schism is healed, Apostate can be traded with for herbs in exchange for blood and organs.

The Soul-and-a-Half Fortress |
Catnip (Душица) is a young girl, and a member of the Soul-and-a-Halves.
She is present in the Fortress throughout the game, and serves as an alternative trading resource alongside her peer Dandy. On Day 1, she will give the Haruspex some Milk and Toast after the "trial" that Notkin invites him to. If the Haruspex accepts her food, he has the opportunity to return the favour by giving her a piece of Toast the following day. The player will earn the In Kind achievement.
If the Haruspex returns with a piece of Toast, Catnip will tell him that she and Tom-Tom always believed in him, unlike Lina and Barrel, and says that the Haruspex is a good person. She'll ask him for a hug, which he can accommodate unless he's died 2 or more times. (This is just one of three hug opportunities in the game, both others being with Lara on Day 3 and Day 11).
Catnip is a variant of the Chickadee NPC.
Trade
Throughout the course of the game, the Haruspex can trade with Catnip.
Notably, Catnip offers the cheapest Fingernails in the game. She always has 2 available for trade.

Chemist (Аптекарь) operates what is effectively a Pharmacy in the front half of Var's place. Items sold and their respective prices are identical to those found in other Pharmacies. Despite his suspicious demeanor, the Chemist will not trade with the Haruspex if his reputation in the Bridge Square is low (however Var will always be available for trade).
After 07:30 on Day 5, Sticky will tell the Haruspex that he was recruited by a chemist in the Atrium district to embalm corpses. If the Haruspex presses him for more information, Sticky will reveal that the chemist wanted Sticky to recruit the Haruspex for the same job. This will lead the Haruspex to the Chemist in Var's Place. The Chemist will insist he is embalming bodies per the Kains' implicit suggestion. Should the Haruspex approach Victor Kain about this, Victor will express disbelief, and ask that the Haruspex to find evidence. The Haruspex is then instructed to mark any houses containing embalmed bodies with a white cross made from chalk.
The Haruspex will find a house on the southern end of the Bridge Square that is so quiet it's remarkable. An upstairs bedroom will contain a number of embalmed corpses, proving the Chemist's claims. When the Haruspex exits the home, its owner begs him not to mark its door, claiming that they will give up their dead in the evening. The Haruspex is able to believe her and leave the home alone, or remain steadfast and mark its door with a white cross. Should the Haruspex mark the house with chalk, he will spare the Bridge Square district from infection the following day, however the Chemist's pharmaceutical services will be lost for the remainder of the game (Var's services will remain available). Whether the Haruspex marks any other houses in the district, or returns to Victor Kain for a follow up dialogue, will not affect the aforementioned outcome.
Chemist shares a model with Looters.

Cricket (Сверчок) is the name given to the children who can be found in the Nutshell sitting by a broken lantern starting from Day 3. Cricket tells the Haruspex the lantern doesnt work, and when asked how long the lantern has been broken, Cricket responds it's always been like that. The Haruspex asks why Cricket keeps the lantern if it was always broken, and they say Fly brought it from a dumpster long ago, but now Fly lives in the Tower and doesn't visit anymore. When asked why they're not in the Tower, Cricket says that Lisa and Mew are still there, and that they're friends.
The Haruspex can then fix the lantern for them using one Metal Scrap. If fixed, the lantern will light up, projecting an image of the Town, the Abattoir, and the Polyhedron under a starry sky on the walls in blue light. Cricket will be astonished, and the Haruspex will say that this is the history of the Town, three different worlds in the palm of one hand. The image projected by the lantern resembles the Changeling's map from Pathologic Classic HD.
Fixing the lantern will grant the achievement Magic Lantern.
Cricket can be a variant of a Tot NPC or a Teensy NPC, depending on who the Haruspex speaks to.

The Soul-and-a-Half Fortress |
Dandy (Пижон) is a member of the Soul-and-a-Halves. He can be found inside of the Soul-and-a-Half Fortress sitting next to Catnip during Act 1 and Act 2. Dandy tells the Haruspex that he and the other teenagers and children like to trade for sharp objects. The Haruspex can then trade with Dandy for items such as Lockpicks.
On Day 4, the Haruspex will find Dandy sitting outside of the Soul-and-a-Half Fortress. He informs the Haruspex that Patches has passed away, despite being treated the day before. Dandy also tells the Haruspex about some puppies that a Soul-and-a-Half, named Biscuit, brought to them and that it's a good occasion, even though one of the pups was stillborn. If Notkin is sick, Dandy will tell the Haruspex that Notkin has become infected and wonders if it is some form of canine disease. The two will discuss the Plague and Dandy offers the Haruspex a map of that day's infected districts made by their scouts, for which the Haruspex will admonish him and the other children for putting themselves in danger.
After this point, Dandy will be found sitting at a crate on the right side of the Soul-and-a-Half fortress. The Haruspex can trade with him for Infection Maps along with the other items that Dandy had available before. As the maps are created by sending out members of the Soul-and-a-Halves, medicine and tinctures will have a high value.
If Notkin dies, Dandy will be in Notkin's spot in the Soul-and-a-Half Fortress. When spoken to, Dandy says that things have become down after losing Notkin and that Jester yowls at night. Many of the Souls have scattered, and he implies that some have returned to the Polyhedron, as he says that he won't be going to the Tower. The Haruspex asks him why he doesn't want to go back and Dandy says the Tower makes everything too easy, that it fulfills every dream and fantasy, and compares it to living on candy. He says "One needs soup, too." The Haruspex asks if the Souls have replaced Notkin with another leader and Dandy tells him they haven't.
Dandy is a variant of the Finch NPC.
Trade
The first Infection Map on Day 4 will be given to the Haruspex for free by talking to Dandy outside of the Soul-and-a-Half Fortress. Afterwards, the Haruspex must trade with Dandy to receive Infection Maps. The value of Infection Maps will increase daily.

Similar to trading with other NPCs or selling to shops, Dandy's regard for weapons decreases with their durability. The listed values reflect trades made at 100% durability.
While Dandy accepts Metal Scraps, Springs, and tinctures for higher values than anyone else in the game, his regard for home-brewed antibiotics and Painkillers is exceptional.

Gnarl (Корявый) is the owner of the Shady Shop, which is a Grocery located in the Warehouses district. Gnarl is the only store owner in the Town who the Haruspex is able to interact with regardless of reputation, as the Warehouses does not have a reputation meter. Although he is in the Warehouses and is located near Bad Grief's Nest, he is not actually affiliated with Grief's gang.
Gnarl's prices for food items are identical to those of other grocery stores in Town. After the dawn of Day 7 at 07:30, when the Inquisitor arrives, he becomes the only grocer who still sells food for money (rather than Inquisitorial Coupons).
When the Haruspex first arrives at Gnarl's store after the arrival of the Inquisitor, Gnarl asks if he is there to bust him for still exchanging food for money. If the Haruspex questions him, Gnarl explains that the Inquisitor has ordered all grocers to only accept candy wrappers and calls her a dictator. The Haruspex asks if he will stay open for business long and Gnarl says he will, if the Haruspex will buy food from him. Gnarl then says that everything will blow over in a week, as the Inquisitor won't stay in the Town forever, and he'll have a vault of wealth by then.
Gnarl is a variant of the Ouzel NPC.

Hardhead (Деляга) is the name given to the grocers that arrive in the Town alongside the Inquisitor.
When the Haruspex first enters a grocery after the arrival of the Inquisitor on Day 7, Hardhead tells him to put away his purse and that they no longer trade for money. When he asks her what she means, she snaps at him that they only trade food for Inquisitorial Coupons now, due to the Inquisitor's decree in an effort to get rid of profiteers. The Haruspex tells her that he thinks it won't be much use, although Hardhead counters that it will at the very least slow them down. When he asks her where he should get Coupons, she tells him to check with the Fund at the Town Hall, since he - as one of the Town's few doctors - is considered 'upper crust'.
The Haruspex can buy food for money from groceries until 07:30 Day 7, when the Inquisitor arrives. After which grocers are immediately replaced by Hardhead NPCs.
It is possible that Day 7's tagline - In which new actors enter the stage, and the old ones reveal themselves in unexpected ways - is referring to how the grocers are all replaced by Hardhead.
Hardhead is a variant of the Dove NPC.

Hunter (Зверобой) is the name given to several children who are part of a Kickstarter backer exclusive in-game quest. In order to find Hunter, the player must have the reward code entered into their game and be playing in a new save file. When the Haruspex receives his lair, he will find the Note to Old Man in the cabinet. It details an invisible cat hiding around the Warehouses which the children wish to catch and make into a Half.
At the Warehouses, the Haruspex will find several children playing near Notkin's Soul-and-a-Half Fortress. Hunter tells him that they are trying to catch a cat and asks him what they should do. The Haruspex will suggest they try luring the cat with Milk. Hunter asks the Haruspex to bring them some milk to help them catch the cat.
After the Haruspex pours milk into the bowl they've laid out, Hunter thanks the Haruspex for his assistance and gives him an Old Shmowder. The Haruspex leaves and can hear a cat's yowl and some thuds. If he returns to the same spot, he will find Hunter and the other children standing around an overturned bowl of milk. Hunter is upset and blames the Haruspex for their inability to catch the cat, stating that the Haruspex's footsteps were too noisy and scared the cat away.
Hunter can be a variant of the Teensy, Tot, or Swift NPC, depending on who the Haruspex speaks to.

Loy (Лой) is a member of the Kin whom the Haruspex first meets during Day 1 in the Hindquarters. Loy offers to sell him a bull for ¤300, although if pressed he will sell it for ¤200. The Haruspex can ask Loy if the bull is stolen, to which Loy replies that it is not, it is simply that "money is needed more than bulls these days". Loy says that the prices will rise two times tomorrow and another four times the day after that, so he wants to stock up on supplies that day.
The Haruspex asks how he knows this and Loy says that his bull told him. Loy then tells the Haruspex that the bull he is offering to sell is also lucky, and that it will tell the Haruspex what to do when the time is right. He explains that the bull will talk on the scariest day of the Haruspex's life and guide him.
If the Haruspex purchases the bull, Loy will bring it to his lair on Day 2. He says the Haruspex will be rewarded for believing him.
Loy is a variant of the Pochard NPC.

Maera (Мэра) is a Doghead.
Starting from Day 4, the Haruspex can find Maera in the Nutshell. Maera asks the Haruspex for a Soul-and-a-Halves leash in exchange for a shmowder. She explains she doesn't need the shmowder as the Polyhedron is infection-free, but she needs the leash. To take up the role of Hound in the Dogheads, she needs one more leash (symbolising a defeated Soul-and-a-Half). This leash can be acquired with trickery or diplomacy, so she is happy to trade one with the Haruspex.
Maera is a variant of the Chickadee NPC with a Doghead mask.

Rat Catcher (Крысолов) is the junior prop master in the Theatre of Death whom the Haruspex meets on his third death, taking the place of Mark Immortell. Amongst her duties, such as selecting skulls for tragic monologues and banana peels for funny falls, she chases away rats.
The Rat Catcher says that she is looking for a little demon that used to live in the Theatre, but has escaped into the big world. She interrogates the Haruspex and thinks that he allowed the creature to escape when he entered the Theatre and didn't close the door properly.
As she is preoccupied with the disappearance of the creature, the Haruspex does not receive any punishments for this death.
Rat Catcher is a variant of the Starling NPC.

Steeper (Топила) is a factory worker who works in pickling. He can be found sitting next to the ferryman at the Factory.
Steeper tells the Haruspex that he knows the Haruspex is being suspected and hunted for the murder of Isidor Burakh. He offers to sell the Haruspex a Revolver for ¤3000, which Steeper had purchased from Bad Grief. The Revolver will have approximately 50% durability and remain available to the Haruspex until the start of Act 3.
If pressed about why Steeper does not give him the Revolver for free, Steeper tells the Haruspex that the Factory has been shut down and he is concerned about his job security and needs to provide for his family. Steeper also says that Big Vlad does not care for his workers, and doubts that the Olgimskys will offer any financial assistance to those who lose their jobs.
Steeper is a variant of the Nightjar NPC.

Aysa Klyonina (Айза Клёнина) is a young woman who is a friend of Yulia Lyuricheva and lives with her in the Trammel.
On Day 6 in the evening at 19:30, she will meet with the Haruspex to inform him that Yulia wishes to speak with him regarding the Inquisitor, who will be arriving the following day. She claims that the Inquisitor's arrival is a trap and it would be best for him to know about it.
Unlike Yulia, Aysa is an engineer, though she helps Yulia with her calculations. She has a voice like iron clanking and a firm handshake.
Aysa is a variant of the Dove NPC. She does not appear in the Trammel after arriving, instead giving the Haruspex the opportunity to speak with Voronika Croy.

Evergreen (Медуница) is a messenger who approaches the Haruspex on Day 2 at 10:00 to impart on him the importance of trading within the Town. The quest received from her is a tutorial on the game's trading mechanics.
Evergreen gives the Haruspex a Thimble and tells him that it is a sign of trust. She says that people share their feelings - warmth, kindness - with each other in the same way they share things. She says that the Town now trusts him and asks if he wants to become part of it. He can protest that he is part of it, but she says he has been away for too long. Evergreen then asks if the Haruspex likes the item, as she chose randomly because nobody really knows him yet.
When the Haruspex asks what she wants in exchange, she tells him it doesn't matter what is exchanged as long as the exchange itself occurred. She then emphasizes that all items are charged with bits of the person carrying them around - the person's warmth and life. Evergreen also mentions that if he shares generously, the Town will remember, and people will like him better. She then gives him the quest, saying that he should go trade with everyone to help the Town remember, accept, and maybe even love him.
The quest requires the Haruspex to trade with a man, a woman, a teenager, and a child. After doing so, his reputation in the Town will increase in all districts.
Whilst trading with townsfolk as part of Evergreen's quest, the Haruspex will receive unique dialogue the first time he trades with someone from each category.
- Men will tell the Haruspex they have carried their item for several days and that it has been difficult. They share their fear, sorrow, and anger, and thank him for easing their burden. The Haruspex can offer his grief, his warmth, or wish that they find his item useful.
- Women will give the Haruspex hope. They say he is the only doctor there now who can treat the wounded and help deliver children. She then says hope often leads to love. The Haruspex can offer to give them a bit of warmth or his grief.
- Teenagers will give the Haruspex some of their luck and mentions that people say they have a lot of it. The Haruspex can offer to give them patience, courage, compassion, care, or wit.
- Young children haven't learned the ritual yet and are confused after trading. They ask the Haruspex about what he has given them and then ask him what he would like in return. The Haruspex can ask for some rest, to leave the Town, or to have his father back, although he will mention it's only a dream.
Trade
Conveniently, Evergreen may be traded with after receiving her quest. However if the player's reputation is too low, they will be unable to trade with her and will have to find another townswoman to trade with to complete that portion of the quest.
While Evergreen is a Starling NPC, her trade preferences are actually those of a Dove. The following table is an example of a Dove NPC's trade preferences.

Scout (Посыльный) is a member of the Soul-and-a-Halves. He is a messenger sent to the Haruspex by Notkin on Day 3 at 11:00. Scout tells the Haruspex that one of their members, Patches, has become ill and will likely die soon. They had already requested the Bachelor's help when he was searching the Warehouses, but he was unable to treat Patches as he believed the medicines the Souls had available were too old and dangerous to be used. Scout calls the Bachelor a fraud for his inability to treat Patches and because he wants them all to stay inside.
Notkin thus sent Scout along with several other Souls out to search for the Haruspex. After relaying the details of the situation, Scout also tells him there is a time-limit, as the Bachelor said Patches would only survive until the evening, before nightfall.
Scout is a variant of the Finch NPC.

Whistler (Свистун) is a messenger whom the Haruspex will meet during Day 1 shortly after learning of Isidor's death (through a conversation with Bad Grief, Lara Ravel, Bachelor or any citizen in front of Isidor's house). If Haruspex does not learn of Isidor's demise, Whistler will begin looking for him after enough time passes (after midday).
He is the driver of the train that the Haruspex rode back to the Town. He tells the Haruspex that the town has turned to murder and is frightened, after the sudden deaths of Isidor and Simon Kain. He also informs the Haruspex that Fingernails can be exchanged with ferrymen for passage throughout the town, and that Townsfolk can help with equipment.
Whistler observes that the Haruspex was alone on the train, claiming he can feel the "living weight", and asserts there was only one human being on the train, even when the Haruspex claims he was not alone.
Prior to conversation with Whistler, no ferrymen exist in the game. After being informed by Whistler that he is suspected of patricide, the Haruspex's reputation throughout the Town's districts will begin dropping rapidly. The reputation will remain neutral if the player avoids the interaction.
Whistler is a variant of the Crow NPC.

Ace (Асса) is the name given to several children involved with the children's cache game that the Haruspex can play during Act 2. They are found gathered on The Cape near the tombs of Nina Kaina and Victoria Olgimskaya.
They admonish the Haruspex for walking so loudly, stating that his stomping walk will wake the Mistresses from their sleep. When asked about the children's caches, Ace says they heard from Sleepy Head already and point the Haruspex to the next children's cache. Ace also suggests heading to the Nutshell once he's obtained a lead on the person who murdered his father.
Ace can be a variant of the Finch, Swift, or Chickadee NPC, depending on who the Haruspex speaks to. However, the Chickadee variant of Ace cannot be spoken to through normal gameplay, as she is sitting too high in the tree.

Goldfinch (Щур) is a young teen that the Haruspex can meet whilst playing the children's cache game during Act 2. They are found south of the Cathedral. Goldfinch expresses surprise that the Haruspex had been invited to play, and quips that the next player they invite might as well be Barley the Thug.
The Haruspex asks them if they've found anything strange and they direct him to a cache located in the Gut. Goldfinch then warns the Haruspex to leave items behind and play the cache game correctly, as the children are following and watching him.
Goldfinch can be a variant of the Chickadee NPC or the Siskin NPC, depending on who the Haruspex speaks to.

Pup (Киса) is a member of the Soul-and-a-Halves. They, along with several other children, meet with the Haruspex outside of the Cathedral during Act 2 to invite him to join their game. They decided to invite the Haruspex because they are also looking for a murderer - the first adult they had invited to their game. They tell the Haruspex that the murderer had left behind certain items related to the Old Man and want his help to find and punish the murderer.
By finding the locations of special caches, the Haruspex may take items from its contents, so long as he leaves behind items of equal value to what he took. Pup gives him the location of two caches as a head start — one north of The Cape, and another south of the Bridge Square, near a collapsed stone bridge.
Pup will also be present in the Soul-and-a-Half Fortress throughout the rest of the game, playing with the other Souls.
Pup can be a variant of Swift, Finch, or Chickadee NPC, depending on who the Haruspex speaks to.

Tatochka (Таточка) is the name given to the children standing in the backyard of the Crucible that the Haruspex can meet during Act 2 when playing the children's cache game.
Tatochka reminds the Haruspex that the children didn't invite him into the game because they liked him and states no one's happy they had to do so. He warns the Haruspex not to let any other adults know about the children's cache game, or else everything will become messed up. They tell the Haruspex the location of a cache in the Gut and remind him to be sneaky about it.
Tatochka can be a variant of the Siskin, Finch, or Swift NPC, depending on who the Haruspex speaks to.

Peter the Nightjar (Козодой) - referred to as just Nightjar in dialogue - is one of several men in the Town who have been hired by Alexander Saburov to hunt for the murderer of Isidor Burakh on Day 1. One of the leads they are ordered to investigate is a creature of Steppe legend, a Shabnak-adyr.
The Haruspex finds Nightjar in the Warehouses, along with another Militia NPC in the Warehouses. He and the other man have cornered a Kayura. When the Haruspex talks to him, Nightjar says that all they did was demand to see the woman's legs to see if she was made of clay.
The Haruspex may either call them scum for harrasing the woman, or ask why they wanted to check her legs, to which Nightjar will explain the legend of Shabnak-adyr. Either way, he will introduce himself later in the conversation as being on duty as a Saburov's man and will ask who the Haruspex is.
If the Haruspex reveals he's Artemy Burakh, Nightjar will say that Isidor saved his daughter and will ask to send his regards. At this point, the Haruspex may become angry at them and tell them to stop harassing the woman and hovering around her, as it was clearly making her uncomfortable. Alternatively, if Haruspex has not yet learned of his father's demise, he can promise to send Nightjar's regards. If Haruspex has already found out that Isidor had been killed, another dialog option will appear instead, where he will say that there is no point in sending regards, as his father is dead.
If instead of revealing his identity, the Haruspex asks who the killer is, Nightjar will say they're looking for Shabnak-adyr and that the creature can only be recognized by having legs made of bones or clay, to which the Haruspex may either reply that he remembers the tale, or jokingly say that it's good they're not looking for ghosts or domovye.
Peter the Nightjar is a Nightjar variant of the Militia NPC. The other unnamed guard is an Ouzel variant of the Militia NPC.

Urmaan (Урмаан) is a Herb Bride who stands by Stanislav Rubin's Apartment on Day 1. She is holding an animal's skull and by doing so, scaring the Townsfolk. Many men can be seen running from her. When asked by the Haruspex what she is doing, she insists that the bones are hers and that she's not hurting anyone by simply holding them. Upon his return later, Urmaan is no longer there, and her bones are abandoned.
The next day, if the Haruspex visits the area, he will find several children running away. When he investigates, he will find a little girl called "Wench" («Баба») sitting beside the bones Urmaan left behind. She questions why he isn't running away and he asks her what she's doing. "Wench" says that she's playing and pretending to be a clay wench. If the Haruspex teases her, she gets upset and complains that she can't get up to stomp her feet at him.
Urmaan is a variant of the Herb Bride NPCs and "Wench" is a variant of the Teensy NPC.

Fingersmith (Щипец) is a member of Bad Grief's gang. On Day 1, the first time the Haruspex's health reaches 0, he is woken up by Fingersmith inside of Bad Grief's Nest. Fingersmith offers him some bread or water and tells him that they die on their own terms, rather than by the hands of others. He then complains that the Haruspex ruined his blanket by coughing blood on it and requests the Haruspex pay him back, along with something extra for saving his life.
Fingersmith explains that initially, he and the others thought the Haruspex was dead and were about to loot him when Crop noticed he was still alive and stopped them because he knew that the Haruspex and Bad Grief are friends. Although they were rewarded by Bad Grief for helping the Haruspex, Fingersmith is still upset about the loss of his blanket and reminds the Haruspex of what he owes.
Afterwards, for the rest of Day 1, the Haruspex will reawaken at a random place in the Town if his health reaches 0.
Fingersmith can be a variant of the Crow or Ouzel NPC.

Kingfisher (Зимородок) is a man who was attacked by three vigilantes for being mistaken for Isidor Burakh's son on Day 1. After witnessing the attack, the Haruspex has the choice between not doing anything and letting the vigilantes kill Kingfisher, or letting them know he is Isidor's son and engaging the three in a fight.
After the fight is over, Kingfisher will commend the Haruspex on his bravery, saying it takes true courage to claim his name today, and call him a hero - not for defending him, but for not fearing the truth. The Haruspex can ask to dress Kingfisher's wounds, which will give Kingfisher a Bandage from his inventory if he has one. If the Haruspex does not have a bandage, Kingfisher says workers carry bandages and tourniquets around since there are lots of injuries at the cannery, and they might be willing to trade some. Giving Kingfisher a bandage does not increase reputation.

The Haruspex will learn that Piecework was stabbed with a Bloody Lockpick after he assisted in burning an innocent Herb Bride alive in the Bone Stake Lot, fearing she was a shabnak-adyr. The Haruspex has the opportunity to either save or kill Piecework — killing him will cause his companion, Shiner, to turn hostile against the Haruspex.
Operating on Piecework introduces the player to surgery game-play. The Bloody Lockpick inside him can be used to pick locks, and is meant to also serve as an introductory tutorial to looting mechanics.

Shiner (Кукиш) is a member of Bad Grief's Gang. He is an accomplice of Piecework and is responsible for dragging him to safety after being stabbed.
He explains to the Haruspex that Piecework was stabbed shortly after burning the Herb Bride in the Bone Stake Lot. He justifies the burning as being afraid of her, believing she was the Shabnak-Adyr, and that they were avenging the death of Isidor Burakh. He blames the Kin for stabbing his companion. He seems to lack basic medical knowledge, such as how to dress a wound or clean up blood. He tries to get the Haruspex to give Piecework alcohol to numb the pain. If the Haruspex does not have a scalpel during the dialog and tells Shiner about that, Shiner will give him a Rusty Scalpel.
If the Haruspex kills Piecework, either accidentally or intentionally, Shiner will attack him, and get killed in the process. If the Haruspex helps Piecework, he will be impressed at his handiwork. Should the Haruspex choose to loot the building that Shiner is in, he will not react as other NPCs do to looting.
Shiner is a variant of the Ouzel NPC.

Agnia (Агния) is a young woman who stands outside of Georgiy Kain's wing of the Crucible on Day 3. She tells the Haruspex that she knows the Judge will allow him to slay outlaws wandering the Town's streets without repercussions. He will, in fact, be rewarded for doing so instead.
She wonders if her boyfriend will take up arms against the outlaws and hopes desperately that he does not. If he is planning on doing so, she is leaving him. She believes that the moment a person decides that murdering any other human - regardless of the guilt or innocence of the individual - is acceptable, something is wrong with them. She does not want to watch one she loves become a killer.
Agnia is a variant of the Oriole NPC.

Blood (Кровь), Nerve (Нерв), and Skin (Кожа) are a group of people standing near the Crucible, gazing up at the roof of the Cathedral on the morning of Day 3.
Blood tells the Haruspex that the crows being so numerous that year is a bad omen, although he will agree that it is beautiful. He tells the Haruspex the gathering of crows means they are waiting for carrion, and being eaten by crows means giving oneself to the sky, rather than Mother Boddho, the Earth. As the Kin revere Boddho and believe the sky is the origin of evil, it is considered a bad way to go.
Nerve tells the Haruspex that according to the Kains, every building is a machine. He says the Cathedral has nothing to do with religion and people claim that it makes time. Nerve proposes that Simon Kain wanted people to feel the flow of time differently within it and planned its construction accordingly, but the idea was simplified due to the rumours. He says that the crows slow time down, which the Kains hate, as they seem to believe complacency and laziness are the worst sins, tantamount to death.
Skin tells the Haruspex that the crows not being noticed by anyone else is a bad omen. She says that since crows are harbingers of death, whomever doesn't notice them will die soon. She says that the three of them - Blood, Nerve, and herself - had been standing there for a long time and many people passed by without stopping to look at the crows. The Haruspex can correct her, as he is now the fourth person who has stopped to look at the crows.
Blood is a variant of the Pochard NPC. Nerve is a variant of the Goose NPC. Skin is a variant of the Chickadee NPC.

On Day 2, the Haruspex may find four Dogheads gathered around the bottom of a Stairway to Heaven. Chara asks the Haruspex which side he is on - the Soul-and-a-Halves or the Dogheads. Chara explains how the two gangs are different and their relationship with the Polyhedron. The Haruspex can ask them what they are doing and discover that they have chased a Soul-and-a-Half who is currently sitting on the Stairway. Chara says they are waiting for him to come down.
All of the Dogheads will be named Chara regardless of which one the Haruspex speaks to.
Chara can be a variant of the Chickadee or Finch NPCs with Doghead masks.

Hatchling (Малёк) is the name given to the children standing outside of Isidor Burakh's House on Day 2 after the Haruspex unknowingly released the Plague.
Hatchling tells the Haruspex that the house must be sick with shingles. They say that Verochka had shingles the year before, which their mother originally thought was a cough, but a doctor - "Grandfather Isidor" - told them it wasn't. Hatchling then says that Isidor was a good doctor and healed Verochka within a week, although she had to cut off her hair. She tells the Haruspex if he ever gets sick, he should find Isidor.
The Haruspex is saddened, as he knows his father Isidor Burakh has already passed away. Hatchling will comment that the Haruspex suddenly appears sad, and he distracts them by asking them about the house again. Hatchling tells the Haruspex that their dad believes it to be soot, but they think their dad is wrong. They say that the house has become sick, and that the houses of the town are alive and can breathe and talk.
Hatchling can be a variant of the Siskin, Swift, Finch, Teensy, or Tot NPC, depending on who the Haruspex speaks to.

Heron (Цапля) is the woman sitting outside of the Shelter next to a water pump on Day 4.
Heron tells the Haruspex that the water pump is broken and that several water pumps all over Town have been destroyed. She says that the Gorkhon's water is too foul for drinking, but they can still use it for washing and cleaning after boiling, since cholera dies in boiled water. The Haruspex asks her who broke it and she says that she doesn't know.
Heron tells him that people have turned into wolves, running around in packs. Aside from destroying pumps, some are looking for the Plague carrier while some are stocking up on kerosene in order to burn houses. She also mentions that she's heard of a "suicidal tagloor", a group of people who don't wait for death because they're too afraid. However, when asked what the word "tagloor" means, Heron says she's not quite sure as it is a word from the Steppe Language. She assumes it means 'community' and the Haruspex corrects her that it means a group of people who share a common fate.

Lefty (Левша) is the name given to the adults standing outside of Isidor Burakh's House on Day 2 after the Haruspex unknowingly releases the Plague. When spoken to, Lefty points out the growths on the house and notes that they seem greasy. He wonders whether or not they are safe to touch, and references the saying "Rough as dutch, safe to touch. Smooth as bone, leave it alone." Lefty then mentions that the phrase was supposed to be used similarly to the phrase "Taking the road less travelled."
The Haruspex asks Lefty what the mold on the walls are and Lefty suggests it could be soot from the Factory. Lefty then decides to leave, stating that breathing in soot can't be healthy.
Lefty can be a variant of the Crow, Ouzel, or Pochard NPC, depending on who the Haruspex speaks to.

Patches (Починка) is a young boy who is a member of the Soul-and-a-Halves. On Day 3, he falls ill and his fellow Souls will seek out the Haruspex to operate on him.
The Haruspex has the opportunity to successfully treat Patches through correct diagnosis, randomly guess the correct diagnosis (or by saving and reloading the game), or to outright fail treatment. He also may choose to let Aspity treat Patches, which will result in her death. However, Patches will eventually die regardless of the outcome of his treatment, even if he is given a Shmowder.
The Bachelor, who had examined Patches in the morning, determined that the boy would only survive until the evening, before nightfall. In accordance with this, the Haruspex only has until 19:00 to attempt to treat Patches before Aspity treats him and dies.

Smokey (Дымок) is a member of the Soul-and-a-Halves. On Day 2, he can be found on the Stairway to Heaven next to the Nutshell. He is being awaited below by four Dogheads, including Chara.
The Haruspex comments on the weird games children in the Town seem to be playing now and asks Smokey if the Dogheads will beat him if he goes down. The Haruspex can offer to help and Smokey says that his Half already went to the Warehouses to call for help.
Smokey says that things used to be peaceful between the two groups. Everyone used to only mingle with their own gang, but recently it's escalated into a war. He mentions the Soul-and-a-Halves made the first attack, but it was because the Dogheads "got under [their] skin". He then tells the Haruspex to leave, as adults have no business in their wars. The Haruspex asks if he's sure that he'll be fine. Smokey says that everything will calm down in the evening when the kids all gather at the Train Station, which is a neutral territory.
He says the children will be at the Passenger Station. They used to gather at the Cargo Station because it was cooler, but it's been boarded off and is too dangerous for the smaller children to get to. When the Haruspex mentions that he wishes to drop by, Smokey gets angry and tells him to stop prying, and says that grown-ups are not welcome where children gather because they ruin everything.
Smokey is a variant of the Finch NPC.

Anna Gofmann (Анна Гофман) is a resident of the Town, and the sister of Andrey Gofmann. She appears in the Day 7 Pantomime under interrogation, alongside others such as Alexander Saburov and Capella. She appears before Aglaya Lilich on behalf of her brother, who shot himself to avoid speaking with Aglaya. She pleads with the Inquisitor to not punish her and others who are there on behalf of dead relatives. Aglaya simply states her request is 'noted' before calling upon Capella to speak.
Anna is a variant of the Dove NPC.

Hogger (Торгаш) is a storekeeper located in the tailor's shop in the Bridge Square. On Day 4, Sticky offers to help the Haruspex obtain the tools needed to fix the brewery in the Lair by robbing Hogger. He justifies this by explaining that Hogger would cheat him and other kids when he acted as a fence for them.
The Haruspex can distract Hogger by acting as if he has come to inspect the store, and later by telling a story about his time as a medical student. The longer he distracts Hogger, the more Sticky will be able to steal from him, as well as the needed tools.
Hogger is a variant of the Crow NPC.

Haruspex's Lair |
Kooseh (Кусэ) is a worm who appears in outside of the Haruspex's Lair on Day 4.
Kooseh, alongside a partner, has gone against the customs of the Kin and the Town and cut open a body in order to extract organs. In doing so they believe they have doomed themselves as Mother Boddho does not appreciate when bodies are improperly cut. Still, Kooseh and their partner have made this sacrifice in order to aid the Haruspex in making "real cures". With the organs the Worms provide, they instruct the Haruspex to use "meat" to make stronger cures as they claim that Isidor Burakh was able to make cures for any ailment with the proper organs and tincures.
Depending on conversation, Kooseh and their partner may give the Haruspex a bottle of blood, a kidney, a liver, or a heart which he may use to brew Painkillers.

Mark Nerchik (Марк Нерчик) is a resident of the Town. He appears in the Day 7 Pantomime, interrogated by Aglaya Lilich, alongside other individuals in town, such as Alexander Saburov and Capella.
In the pantomime, he confesses that he and his gang - named Kittykat, Stilts, and Knives - were robbing individuals after the plague broke out, and had plans to rob the Bachelor. Aglaya sentences him to the Northern Goldmines for ten years as punishment.
Mark is a variant of the Goose NPC.

Olkho-o (Ольхо-о) is a worm who appears in the Hindquarters alongside a group of the Kin on Day 5.
Olkho-o is beside the bull that the Kin are parading through the streets. They are allowing the bull to say goodbye to the Town, allowing the Town to bind the bull with love. Olkho-o asks the Haruspex to aid the Kin is cutting the bull open at the Ragi Barrow as they require a butcher to do so. The Haruspex can accept or decline the invitation.
If the Haruspex accepts, he will be told to come to the Ragi Barrow at nightfall to slaughter the bull. After slaughtering the bull Olkho-o will give the Haruspex a bottle of the bull's blood, claiming that it will make him resistant to the Sand Plague, as bulls cannot be infected.

Stump says that he came by the house he's found in to warm up and just started to play the piano, despite never having played music before.

Uta is rude to the Haruspex, threatening to scream and telling him to leave (she uses a number of ways to tell him this). She claims to have written the melody herself.
Uta is a variant of the Chickadee NPC.

Bobhead (Стриженый) is the name of the men found in the Shelter on Day 10. The Haruspex asks them what happened and Bobhead tells him about Lara's plan to kill the Commander. He says that she wants to get revenge for what happened to her father, as she believes Block was responsible for his death.
Bobhead can be a variant of a Nightjar Militia NPC or Crow NPC, depending on who the Haruspex speaks to.

Isidor Gasar (Исидор Газар) is a resident of the Town. He appears on Day 11, captured by bandits in the Broken Heart pub. He insists he is Artemy Burakh, and will only admit to his own name after pressing from the Haruspex. He explains that he is the next actor in the Theatre of Death.
When questioned on the courier sent by the Inquisitor, Gasar says he tried to ambush him so the orders were not delivered to the general. He claims that those were his stage directions.
The Haruspex can resolve this encounter peacefully, either conceding that Gasar can succeed him or saying that he will succeed Gasar. The Haruspex can end it with Gasar becoming hostile if he states the world isn't big enough for the two of them. If killed he is able to be looted, though doing this will incur a reputation loss. Given that he is to succeed Artemy Burakh, Gasar holds items that the Haruspex would have, such as herbs, medicine, water bottles, nuts, and a rusty scalpel. These items are randomised in both type and quantity.
Though in dialogue he is referred to as "Isidor Gasar" in his name card his surname is instead spelled as "Ghazar".
Gasar is a variant of the Nightjar NPC.

Nota (Нота) is a Doghead.
On Day 9, the Haruspex can find Nota outside the Cathedral. She will only appear if Grace is dead. Nota tells the Haruspex that the children have been calling him when he visited the Cathedral, to warn him away from where they are meeting on the Polyhedron. She explains they are meeting on the tower where the air is clean so they will be safe from the Plague.
Nota is a variant of the Chickadee NPC with a Doghead mask.
